The Case Manager works under the direction of the Program Director and is responsible for aiding in the delivery of case management and clinical treatment services and assistance in coordinating program operation.
RESPONSIBILITIES:
- Ensure that all youth receive services while in care including but not limited to mental health, medical, recreation, education, and other support services
- Maintain accurate, and current information on foster children in care, while ensuring ORR reporting requirements are met.
- Coordinate ongoing searches and connection with family or significant adult sponsor (Instead of HV language)
- Assist the Clinician/Consultant in providing other case management services, including scheduling client’s activities, and making appropriate contacts with referring workers, program staff, etc.
- Assist the Clinician/Consultant in Case Reviews by sending out invitations, compiling test results, attending, and participating in Case Review Meeting discussions, and writing summaries.
- Assist the Clinician/Consultant in processing case related information, including profile sheets for new admissions, preliminary work on service plans, follow-up calls for discharged clients, maintaining up-to-date client files, etc.
- Assist the Clinician/Consultant with linkages with the client’s Goal Worker and other program staff including the Nurses, Wellness Specialists, School Coordinator, etc.; work to keep all service providers informed regarding changes in programming, special needs, etc.
- Lead or co-lead a milieu group, as requested.
- May be required to specifically monitor the progress of certain clients, as assigned. This may include work with clients on community living skills.
- Regular participation in supervision meetings and team meetings for the sharing of ideas, feedback, and department planning.
- Responsible for quality assurance compliance, including but not limited to participating in peer supervision, peer utilization reviews, compliance with service delivery criteria and reporting requirements for each individual served.
- Responsible for timely Quality Assurance compliance as related to employee’s roles and functions.
- Other duties as assigned
QUALIFICATIONS:
- Bi-lingual English and Spanish preferred
- Bachelor’s degree is required.
- LBSW or ability to be licensed is preferred.
- Prior experience working with refugee and/or immigrant populations preferred.
- Minimum of one year experience as a Case Manager or a Youth Treatment Specialist.
- Excellent verbal and written communication skills.
- Federally funded program requiring the following vaccinations:
- Varicella: two doses of varicella vaccine (at least 4 weeks apart)
- OR born in the United States before 1980 with a reported history of chickenpox (verified by an adult present at the time of illness)
- OR titers indicating varicella immunity
- OR documentation from healthcare provider verifying previous infection.
- MMR: Two doses of MMR vaccine (at least 4 weeks apart)
- OR titers indicating immunity to all three diseases (Measles, Mumps, and Rubella)
- DTaP or DTap
- Complete primary vaccination series. If primary vaccination series completed in childhood, one DTaP dose in adulthood.
- Influenza: Single dose annually during flu season.
- Varicella: two doses of varicella vaccine (at least 4 weeks apart)
- Subscription to the Statement of Faith of Wedgwood Christian Services and agreement with the goal, purpose, and objectives of the agency.
HOURS OF WORK: M-F 8:30-5 nights and weekend and on call availability
