The Lead Case Manager is responsible for coordinating case management and safe and timely release services, training new case managers, and supervising the work of other case manager(s) and case aide(s). The Lead Case manager will be responsible for timely admissions and discharges from the program.

RESPONSIBILITIES:

  • Ensure that all youth receive services while in care, including, but not limited to, mental health, medical, recreation, education, and other support services.
  • Maintain accurate and current information on children in care, while ensuring ORR reporting requirements are met.
  • Coordinate ongoing searches and connection with family or viable adult sponsor.
  • Assist the Clinician/Consultant in Case Reviews by sending out invitations, compiling test results, co-facilitating in Treatment Team Meeting discussions, and completing/compiling pertinent documentation.
  • Lead admission and discharge process for clients within the program.
  • Assist the Clinician/Consultant with linkages with the Education and Life Skills Specialist and other program staff including Nurses, Wellness Specialists, etc; work to keep all service providers informed regarding changes in programming, special needs, etc
  • Lead or co-lead a milieu group, as requested.
  • May be required to specifically monitor the progress of certain clients, as assigned. This may include work with clients on community living skills.
  • Regular participation in supervision meetings (with the director and supervisees) and team meetings for the sharing of ideas, feedback, and department planning.
  • Responsible for quality assurance compliance, including but not limited to participating in peer supervision, peer utilization reviews, compliance with service delivery criteria and reporting requirements for youth served.
  • Responsible for timely Quality Assurance compliance as related to employee’s roles and functions, including leading collaboration with the QA/PSA Compliance Manager on proactive or corrective actions.
  • Participate in on-call rotation for urgent client needs and reporting.
  • Other duties as assigned.

QUALIFICATIONS:

  • Bi-lingual English and Spanish required
  • Master’ Degree in behavioral services preferred.
  • Bachelor’s degree with three years of case management experience is required, one year of which being in refugee-serving case management role.
  • Previous supervisory experience strongly preferred.
  • Excellent verbal and written communication skills.
  • Federally funded program requiring the following vaccinations:
    • Varicella: two doses of varicella vaccine (at least 4 weeks apart)
      • OR born in the United States before 1980 with a reported history of chickenpox (verified by an adult present at the time of illness)
      • OR titers indicating varicella immunity
      • OR documentation from healthcare provider verifying previous infection.
    • MMR: Two doses of MMR vaccine (at least 4 weeks apart)
      • OR titers indicating immunity to all three diseases (Measles, Mumps, and Rubella)
    • DTaP or DTap
      • Complete primary vaccination series. If primary vaccination series completed in childhood, one DTaP dose in adulthood.
    • Influenza: Single dose annually during flu season.
  • Subscription to the Statement of Faith of Wedgwood Christian Services and agreement with the goal, purpose, and objectives of the agency.

HOURS OF WORK: Monday-Friday business hours, participation in on-call rotation required
