EDDY VISITING NURSE & REHAB ASSOCIATION


Eddy Visiting Nurse & Rehab Association has a rewarding opportunity for a part time Social Worker responsible to manage a caseload of patients requiring social work support through in-home visits. Some patients/families are supported as well through virtual/phone based social work support either in place of in home visits or in combination with in home visits.


Manages a caseload of patients in their homes:

  • work with an interdisciplinary care team of nurses, therapists and aides
  • conduct a comprehensive assessment of patient/family needs
  • link individuals to the community resources they need to remain in their home or move to a higher level of care if necessary
  • Once trained, enjoy the independence of starting and ending your day from home with a flexible work schedule
  • Be on the front-line of the transitions in health care from inpatient settings to the community – home health care is the future!

Work schedule is flexible.

Requirements:

  • NYS Social Work Licensure, minimum of 1 year of relevant experience in home care, the medical field, work with seniors, etc. required.


Eddy VNA provides a comprehensive orientation to home health care. Eddy VNRA provides a supportive work environment with access to social work supervision, in-depth computer training, assistance obtaining insurance authorizations, support staff who are only a phone call away, and more!

Pay Range: $30.00 - $43.08

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
