Description:
The RN Clinician – Cardiology oversees and coordinates care for patients within the cardiology specialty for an assigned caseload. This role ensures patients and families receive appropriate, timely, and well‑coordinated care throughout the continuum of health services. The clinician facilitates resource utilization, promotes efficient patient progression, and supports high‑quality, cost‑effective care while serving as an educator, advocate, and clinical resource.
Essential Functions
- Completes comprehensive assessments of patients’ physical, psychological, social, environmental, financial, and functional status, including past and current treatment plans.
- Develops patient‑specific goals, treatment strategies, and provider options.
- Collaborates with clinical teams and medical staff on performance improvement initiatives, including assessing, planning, implementing, and evaluating outcomes for designated populations.
- Serves as a clinical expert for patients within the cardiology population, providing internal education and consultation.
- Uses critical thinking and problem‑solving skills to analyze data and formulate individualized case management plans.
- Continuously monitors and revises care plans in coordination with the interdisciplinary team.
- Communicates effectively with patients, families, and healthcare team members, verbally and in writing.
- Participates in developing and implementing policies, procedures, and quality improvement efforts for assigned caseload.
- Builds strong, collaborative relationships with physicians and healthcare team members; promotes collegial relationships across all customer groups.
- Acts as a patient and family advocate, coordinating team conferences and facilitating communication.
- Provides education to promote high‑quality care within the cardiology specialty.
- Functions as a liaison, negotiator, and facilitator between patients, providers, and third‑party payers.
- Establishes goals and metrics for monitoring care quality in alignment with industry standards.
- Performs other related duties as assigned.
Qualifications
Education
Required:
- Graduate of an approved school of nursing.
- Bachelor of Science in Nursing (BSN) OR two years of practice in specialty AND specialty certification (e.g., OCN, CNRN, CCRN‑Pediatrics, or a certification aligned to the specialty area practiced).
Preferred:
- Bachelor of Science in Nursing (BSN).
Credentials / Licensure
Required:
- Current Registered Nurse (RN) license in the State of Michigan.
Experience
Required:
- Minimum of three years of nursing experience.
Preferred:
- Previous case management experience.
- Specialty‑specific experience depending on patient population and care environment.

Required:
- Strong critical‑thinking skills with the ability to guide patients across a continuum of care.
- Excellent communication and facilitation skills.
- Ability to prioritize and manage a complex caseload amid frequent interruptions.
- Advanced teaching and communication abilities (verbal and non‑verbal).
- Demonstrated expertise in caring for the identified patient population.
- Ability to lead practice improvement through change management and professional influence.
