Position Overview - Day 1 Benefits & 15K Sign-On Bonus:
As a Home Care Registered Nurse (RN) at Trinity Health At Home, you’ll deliver one-on-one, high-quality care to patients in the comfort of their homes. Using advanced technology and your clinical expertise, you’ll assess, plan, and manage individualized care that promotes healing and independence. Schedule: Friday-Sunday (work 36 hours, get paid for 40 hours!)

What You’ll Do:
Provide skilled nursing care, assessments, and interventions in the home
Serve as case manager and patient advocate
Collaborate with interdisciplinary care teams
Educate patients and families on care plans and self-management

Minimum Qualifications:
Active RN license in Michigan
2+ years of clinical nursing experience (Home Health preferred)
Epic or Home Care EMR Experience
Strong communication, assessment, and organizational skills
Compassionate, dependable, and mission-minded
Valid driver’s license and reliable transportation required
