Trinity Health Alliance of Michigan (THAM) is seeking a mission-driven Skilled Nursing Facility (SNF) Coach to join our High-Performing Network team. This visible and impactful role is dedicated to coaching and partnering with Skilled Nursing Facilities (SNFs) to improve patient outcomes, streamline care delivery, and enhance performance in alignment with the Quintuple Aim.
The SNF Coach will serve as a trusted consultant and educator to SNF staff and administrators—building relationships, analyzing performance data, and implementing improvements that directly impact patient care and system efficiency. Your work will support and elevate THAM's position as a leader in clinical integration and value-based care.
Drive Quality & Performance
Conduct SNF site audits and maintain performance tracking databases
Provide coaching and training based on High-Performing Network SNF Scorecard metrics
Lead performance improvement plans and root cause analyses
Coach & Educate
Facilitate regular in-person coaching sessions with SNF teams
Deliver education on clinical integration, quality initiatives, and care pathways
Provide SNFs with actionable strategies to reduce readmissions and length of stay
Monitor & Support
Develop and implement tools for tracking and improving outcomes
Assist SNFs with system access and training (e.g., Epic CareLink)
Provide ongoing education on THAM initiatives, requirements, and scorecard benchmarks
Collaborate & Lead
Partner with SNF administration, Post-Acute Care Coordinators, and THAM leadership
Support rollouts of THAM programs, policies, and compliance standards
Promote adoption of best practices across all SNF partners
Bachelor’s degree in healthcare, healthcare administration, or related field
Experience in post-acute operations, SNF management, or healthcare quality support
Strong ability to build and maintain partnerships with SNF leaders and care teams
Exceptional communication and presentation skills, including comfort with senior leadership
Proficiency in Microsoft Office and the ability to learn clinical information systems
Demonstrated initiative, follow-through, and adaptability in a fast-paced environment
Must be able to travel locally to SNFs at least 50% of the time
