Job Title: Skilled Nursing Facility Coach
Location: Trinity Health Alliance of Michigan (THAM) – Regional

Trinity Health Alliance of Michigan (THAM) is seeking a mission-driven, relationship-oriented Skilled Nursing Facility (SNF) Coach to support and strengthen our High-Performing Network of Skilled Nursing Facilities. This highly visible role is dedicated to coaching and collaborating with SNF staff and administrators to improve patient outcomes, optimize efficiency, and elevate the patient experience.
The SNF Coach plays a critical role in driving performance improvement aligned with the THAM SNF Scorecard and the broader goals of the Quintuple Aim. Through data-driven analysis, proactive education, and consistent partnership, the Coach will implement targeted improvement strategies and reinforce Trinity Health’s commitment to exceptional care.
Performance & Outcomes Support
Drive performance on THAM scorecard metrics by supporting SNFs with data audits, analysis, and actionable insights.
Lead site coaching sessions to review outcomes, analyze trends, and identify improvement opportunities.
Collaborate on performance improvement plans with SNF leaders and interdisciplinary staff.
Engagement & Relationship Building
Build and maintain strong relationships with SNF administrators and care teams.
Provide regular coaching, training, and feedback to help facilities achieve and maintain High-Performing Network status.
Educate SNFs on Trinity Health initiatives, clinical integration expectations, and reporting criteria.
Monitoring & Data Analysis
Track SNF performance using structured monitoring tools and provide clear, timely feedback.
Prepare reports on readmission trends, root cause analyses (RCAs), and high-risk patients.
Develop and disseminate educational materials, alerts, and best practices.
Training & Onboarding
Conduct onboarding and orientation for new SNFs and provide ongoing support for staff.
Train teams on tools including Epic CareLink and internal systems.
Identify and share high-impact clinical and administrative practices.
Compliance & Collaboration
Stay up to date on applicable Federal, State, and local regulations, as well as Trinity Health policies and procedures.
Support other strategic and compliance-driven projects as assigned.
Bachelor’s degree in healthcare, healthcare administration, or related field; or equivalent experience in post-acute operations or provider support.
Demonstrated ability to build strategic partnerships and engage leadership across healthcare settings.
Exceptional verbal and written communication skills, with experience presenting to senior leadership.
Strong problem-solving and decision-making skills, with a collaborative mindset.
Proficiency with Microsoft Office Suite and comfort learning clinical information systems.
Ability to travel locally at least 50% of the time to SNF sites.
