Description:

Pharmacy Technician position that supports our Transition of Care Program.

Pharmacy Technician will attend rounds on certain floors of the hospital and identify those who would be good candidates for a Comprehensive Medication Review by the embedded pharmacist at their primary care clinic. After identification the technician will meet with patients to describe the service and schedule appointments for patients with the pharmacist after discharge. Additionally, the technician will perform outreach for educational sessions with our pharmacists who perform education on anticoagulants. Finally, help to support our team of embedded pharmacists with assistance with rescheduling missed appointments.

What the Transition of Care Technician Pharmacy Tech will need:

  • Minimum High school education or equivalent.
  • Preferred Completion of a formal Pharmacy Technician training program; or two years of college with emphasis in math, science, or medical field applications.
  • Minimum Licensed as a Pharmacy Technician in the State of Michigan.
  • Preferred Active status as a Certified Pharmacy Technician (CPhT) by the Pharmacy Technician Certification Board (PTCB).
  • Minimum Two years of pharmacy work experience. At least one year as a Pharmacy Technician III with consistent experience making simple and complex IV Admixtures (for inpatient pharmacy work locations).
  • For Medication Historian positions: 6-12 months successful performance in medication historian role after training (i.e., at Tech III classification). During this time, the medication historian should have progressed to and be practicing at a role-model level of performance considering both productivity and quality of work.
