Make a meaningful difference from the very first visit.
As a Hospice Admissions RN, you will be the compassionate bridge between hospital, home, or facility and our hospice care team—bringing comfort, clarity, and reassurance during one of life’s most vulnerable transitions.
Serve as the first clinical point of contact for patients and families referred to hospice care.
Conduct comprehensive nursing assessments and determine eligibility for hospice admission.
Collaborate with physicians, hospital staff, and interdisciplinary hospice teams to coordinate seamless care.
Provide compassionate education on the hospice philosophy, services, and next steps to patients and their loved ones.
Ensure timely and accurate completion of admission documentation per state and federal guidelines.
Current RN license in the State of Maryland
Minimum of one (1) year of nursing experience; hospice or palliative care experience preferred.
Strong clinical assessment, communication, and organizational skills.
Ability to work independently in a fast-paced, emotionally sensitive environment.
Valid driver’s license, auto insurance, and reliable transportation.
