We’re seeking a Contingent/PRN Hospice Nurse Practitioner to join our compassionate, interdisciplinary team. In this flexible, as-needed role, you’ll provide advanced care and comfort to patients and families facing serious illness in their homes or residential settings.
Conduct home visits for hospice patients to assess, manage, and optimize pain and symptom control
Collaborate with hospice medical directors, RNs, social workers, and chaplains as part of a holistic care team
Provide clinical expertise for admission evaluations, recertifications, and end-of-life decision-making
Support families and caregivers through advanced illness education and compassionate presence
Document accurately and in accordance with compliance and hospice standards
What You’ll Bring:
Current Nurse Practitioner licensure and certification (ANCC/AANP)
Minimum of 1 year NP experience, preferably in hospice, palliative, geriatric, or internal medicine
Valid driver’s license and reliable transportation
Compassion, professionalism, and the ability to navigate sensitive conversations
DEA license (or willingness to obtain, depending on state/regulations)
Flexibility: Work PRN and make a difference on your schedule
