We’re seeking a Contingent/PRN Hospice Nurse Practitioner to join our compassionate, interdisciplinary team. In this flexible, as-needed role, you’ll provide advanced care and comfort to patients and families facing serious illness in their homes or residential settings.

What You’ll Do:

  • Conduct home visits for hospice patients to assess, manage, and optimize pain and symptom control

  • Collaborate with hospice medical directors, RNs, social workers, and chaplains as part of a holistic care team

  • Provide clinical expertise for admission evaluations, recertifications, and end-of-life decision-making

  • Support families and caregivers through advanced illness education and compassionate presence

  • Document accurately and in accordance with compliance and hospice standards

What You’ll Bring:

  • Current Nurse Practitioner licensure and certification (ANCC/AANP)

  • Minimum of 1 year NP experience, preferably in hospice, palliative, geriatric, or internal medicine

  • Valid driver’s license and reliable transportation

  • Compassion, professionalism, and the ability to navigate sensitive conversations

  • DEA license (or willingness to obtain, depending on state/regulations)
