As a Registered Nurse Case Manager, you’ll be a vital part of our interdisciplinary team, helping patients transition safely and comfortably to the next level of care. This is a meaningful role where your clinical expertise and empathy will directly impact patients and their families during critical moments.

✅ What You’ll Need:

Current RN licensure in the State of Michigan
Four years of recent, relevant clinical experience preferred
Strong communication and care coordination skills
A heart for service and a collaborative spirit

Coordinate and facilitate patient-centered care plans
Conduct utilization reviews and ensure compliance with care standards
Meet with patients and families to support safe discharge planning
Promote optimal outcomes within the boundaries of each diagnosis
