Description:
Develops, coordinates, and manages quality and medication use improvement initiatives to assure safe, appropriate, effective and
efficient medication use processes within the pharmacy and health-system. Serves as a liaison between pharmacy, nursing, medical
staff, other healthcare professionals, and corporate pharmacy services in order to ensure safe and standardized medication use
processes. Coordinates activities, leads staff, and manages systems related to quality and medication use improvement to ensure that
the medication use process meets or exceeds the reasonable expectation of customers. Supports the team by staffing as needed.

Education
Minimum: Doctor of Pharmacy
Preferred:
Credentials/Licensure
Minimum: Pharmacist license and controlled substance license issued by the Michigan Board of Pharmacy.

Related Experience
Minimum: Pharmacy Practice Residency (PGY-1) plus three years acute care related experience or five years of experience
with progressive increases in responsibility in the absence of the PGY-1 residency. Experience with
medication safety, medication use improvement, and application of process improvement systems (Lean, 6
sigma)
Preferred: Experience leading projects or initiatives for focused improvements in medication safety, medication use, and
pharmacy process improvement.
Other Knowledge, Skills and Abilities
Minimum. Strong interpersonal communication skills. Ability to analyze complex situations. Demonstrated application of
process improvement tools (lean, 6 sigma) and medication safety principles (ISMP assessments, root cause
analysis, Just Culture algorithms, etc.)
Preferred: Demonstrated personal development in Lean principles applied to healthcare/pharmacy situations.
- Manages the medication error reporting process including peer review, investigation, and analysis. Provides reports to appropriate department and committees on error occurrences.
- Identifies best practices for medication safety. Facilitates process and system changes to reduce the likelihood of occurrence and reoccurrence of error. Monitors the effectiveness of changes made to reduce error.
- Communicates and collaborates with physicians and other clinicians to develop drug therapy policies, protocols, and guidelines.
- Prepares reports and documents for P&T action including formulary drug reviews, medication usage evaluations, policies, and guidelines for drug therapy.
- Coordinates departmental and medication use performance improvement. Facilitates data collection, analysis, and reporting to the appropriate committees.
- Facilitates integration of Lean principles into pharmacy department operations. Provides Lean content expertise for Pharmacy Services.
- Coordinates Pharmacy Lean processes and supports process improvement. Works with the Director of Pharmacy for development and constant improvement of Pharmacy Services Lean Management Structure and oversight of Pharmacy Services process excellence work.
- Participates in departmental, multidisciplinary, hospital and health-system committees as leader, member, or consultant.
- Provides education and information on medication safety and drug therapy to patients, pharmacy staff, physicians, nurses, students, and other clinicians.
- Assists in preparation for regulatory and accreditation review.
- Coordinates the monitoring and evaluation of departmental and medication use policies, procedures, and practices to meet accreditation and regulatory requirements.
- Encourages a learning environment and participates in precepting of students and residents.
- Supports the team by staffing as needed.
- Performs other related duties as assigned.
