Corewell Health West is seeking to hire a board certified/board eligible Family Medicine or Internal Medicine/Pediatrics physician to start up a new Value-Based Primary Care Clinic opening late Fall 2024 to serve the needs of the Boston Square Market residents , and surrounding communities with the collaboration of the local non-profit Amplify GR. in Grand Rapids, MI. The new clinic space will have 6 exam rooms, one procedure room, several flex spaces, and a large classroom which will be used for patient education, cooking classes and community education. This clinic will be decorated with the assistance of local community artists.
This location will provide Value-Based Primary Care, lifestyle medicine, Healthier Communities services, financial counseling, health insurance literacy education, workforce development, language services, and virtual care using TytoCare technology.
The ideal physician candidate is one who has experience and passion for treating patients with diverse racial and ethnic backgrounds that have limited resources. In collaboration with Amplify GR, this practice plans to eliminate health disparities by promoting a healthy lifestyle with this underserved community.
