Registered Nurse - Greater Grand Rapids Area
Care Plus TLC, powered by Axios HR, provides skilled, knowledgeable in-home caregivers across West Michigan. Our service expertise is providing in-home caregiving services in the comfort of your own home while helping to reduce hospitalization costs, as well as providing ongoing needs assessments and analysis to lower costs without compromising the quality of care.
We are looking to expand our medical team by adding a Registered Nurse to our Grand Rapids team. This individual will work autonomously to treat our patients in the comfort of their own homes. Our RNs use clinical judgment skills to access and implement treatment plans best suited for their patients. This person will maintain high professional standards, expectations, and processes in all settings that align with Axios HR's Mission, Vision, and Core Values.
Responsibilities of the Licensed Practical Nurse:
- Complete comprehensive assessments, collecting data in an ongoing and systematic manner, focusing on age, physiologic, psychological and cognitive status of the patient. Completes, reviews and evaluates the data collected by caregivers.
- Performs basic patient care procedures/techniques competently, including the administration of medication. Organizes patient care effectively.
- Develops a plan of care that prescribes nursing actions to achieve patient goals.
- Identifies patient/family learning needs and implements appropriate process from admission to discharge.
- Assess physical and developmental needs.
- Document history and physical/emotional needs.
- Review vital signs to detect deviations from normal.
- Analyze assessment data to determine patient challenges.
- Document expected outcomes and plan of care.
- Identify expected outcomes individualized to the patient.
- Utilize patient/family input and consider values, ethics, and cultural factors.
- Develop a plan prescribing interventions to attain expected outcomes.
- Incorporate clinical pathways and develop individualized care plans.
- Consider data from other healthcare team members.
- Implement the identified plan and administer prescribed medication and treatment.
- Utilize evidence-based interventions.
- Ensure patient comfort and safety.
- Evaluate progress toward expected outcomes.
- Assess the effectiveness of interventions and patient response.
- Document patient response and progress.
Qualifications Responsibilities of the Licensed Practical Nurse:
- Bachelors degree in Nursing required.
- Must be currently licensed and registered in Michigan.
- Must have a current Michigan driver's license.
- One (I) year of skilled nursing experience is preferred.
- CPR certification must remain current during employment.
- Able to work onsite Monday- Friday, 8:00 am to 4:30
