Position Purpose:
The Home Care Nurse Resident is a recent graduate and newly licensed registered nurse who is enrolled into an individualized residency program. The residency program is designed to transition the new graduate safely and competently into professional practice. The program provides a pathway with targeted phases and defined expectations. The goal is to advance the resident from novice to advanced beginner registered nurse, prepared to transition to a primary care nurse by the end of the residency.

Effectively creates and disseminates written and oral communications that clear, understandable, accurate, timely, and professional.
Promotes teamwork by keeping others informed, collaboratively participating in team projects & group decision making, by resolving conflicts in an open, direct, and equitable manner, and through conscious solicitation & provision of feedback
Acts responsibly, and consistently in addresses routine operational accountabilities and calmly and constructively reconciles unanticipated events and crisis situations.
Responsible for meeting/exceeding all established common goals and clinical outcomes.
Responds timely to documentation inquiries/alerts and agency communications
Align clinical pathways in accordance to standard work.

Minimum Qualifications:
Graduate of an approved Nursing education program, BSN preferred.
Current Registered Nurse licensure in the state of practice.
Strong computer and technology skills required.
Must have current Driver’s license and reliable transportation.
Ability to consistently demonstrate commitment to the mission and Organizational Code of Ethics, and adhere to the Compliance Program.
